How do I change my last name on the Texas Nurse Aide Registry?

I got married and I need my last name changed on the Texas CNA registry.

I need a copy of my Texas CNA certificate because I lost the original.

You must complete a form requesting the correction. The application you need is “Texas Nurse Aide Registry Request for Reprint of Certificate and/or Correction to Nurse Aide Registry Data”. You may find the form and instructions here.

The Nurse Aide Registry has implemented a new online system called Credential Manager. Before completing this form, you are required to register in the new system. Once you have registered and got your identification number, include it the at the top of the form.

Attach a legible photocopy of a picture identification that shows your birth date and the correct spelling of your name, your certificate and a photocopy of your marriage license, portion of divorce decree showing the name change or other court order indicating a name change and other documents as required. Make sure to sign the form.

The completed application along with the required documents need to be sent to the Texas Nurse Aide Registry at the address listed at the bottom of the form. The Texas Nurse Aide Registry will return without action incomplete requests and requests without required documents. Tampering with or attempting to falsify a government record such as a nurses aide certificate is a third-degree felony punishable by up to 10 years in prison and a $10,000 fine.
