If you meet one of the following criteria you are eligible for a nursing assistant license in Alaska: have successfully completed a state approved nursing assistant training program not more than two years prior to applying for license, hold a nursing license from any state of the United States or a province of Canada, show proof of completing at least one year of nursing education within the last five years or have graduated from a nursing training program in a foreign country other than Canada.

Message: If you have lost your CNA certificate and wish to obtain a new one you need to complete and submit a form usually titled Request of Reprint of Certificate. Such a form may be obtained by calling the CNA registry in your state and requesting the document, or by accessing and printing the online form. Good luck!
