You must send the Registry a completed Application for Renewal as a Certified Nurse Aide. The renewal form doesn't require your employer to complete or mail the form. You can complete and mail the renewal form yourself. All you need from your employer is their signature verifying that you did work there at the dates specified. The renewal form and instructions may be printed from the registry web site or you may request a form via the Interactive Voice Response System at 678-527-3010 or 800-414-4358.
