My license is dated wrong. How do I correct it?

Had to take courses over because my license lapsed. Passed exams on July 13th, 2016 and it came with original dates of my first license. How do I correct it?

In order to correct the mistake you need to contact the CNA registry of your state. They will let you know if you need to submit any additional documents as proof or whatever else you need to do. You may find contact information for all state registries here.
