The Washington State Department of Health does not have a list of charges that disqualifies you from any credential. They look at everything on a case-by-case basis. When you apply with them you would be required to produce certified judgment and sentencing court documents and a letter of explanation in your own words of the events and the outcome. For additional guidance, you may contact the Washington State Department of Health at 360-236-4700.
