You may download the form and instructions for completing the Iowa direct care worker registry application from here. You complete only Section One, while Section Two is completed by a hiring entity, if there is one. If there is no hiring entity, Section Two is left blank. If you have a registry card/certificate from the other state, make a copy and return it with the form. If not, make sure it is noted on the form in what state you were originally certified.

The form can be returned by either mail or fax with that information identified on the form. Once received in the office, the information will be verified with the other state. Reciprocity is generally processed within one or two working days of receipt. For additional information or guidance you may contact the Iowa Direct Care Worker Registry at 515-281-4077.
