A duplicate or replacement CNA certificate can be obtained as long as the current fee is honored. The nurse aide certificate is usually mailed within two or three weeks. Duplicate certificates are usually requested when the original was lost or damaged. A nurse aide who wishes to change the name on her certification needs to provide a written request with the new name, and her license number. A copy of the legal document showing the change of name is also necessary.
